You’re on your way.
Thank you for reaching out to Maine Dementia Partners. Marjorie will contact you to confirm your consultation. In the meantime, please call if you have a more urgent need.
How do I prepare for an initial consultation?
Gather a current medication list, your loved one's recent medical visits, and any reports from neurology or memory testing. Make notes on the behaviors, decisions, or moments that prompted you to reach out. Identify the family member who knows the day-to-day details best. We will guide you through everything else after you book.
Who else should be on the call?
We recommend including the primary caregiver and any family member who participates in care decisions. For long-distance families, include the local sibling and the out-of-state coordinator. Spouses, adult children, and durable power of attorney holders all benefit from being on the call so everyone hears the same recommendations.
